Have your care provider (physician, midwife, etc.) to fax a referral form to 780-460-4550. Alternatively, they can send a general referral letter but please ensure they include contact and health information for both the breastfeeding parent and their infant. Lactation is a two person relationship (parent & infant), so it is critical for a complete assessment that we see both of you. Referrals are accepted from physicians, registered midwives, nurse practitioners, audiologists, chiropractors, podiatrists, dentists, optometrists & physical therapists. 

 

After we have processed your referral form, you will receive an invitation to our secure online messaging program called Pomelo Health. If you already use Pomelo for another office, please ensure to use a NEW email when signing up for pomelo with our office. This is necessary for you to be able to return online questionnaires if needed or for online booking. When a referral is received, it will be processed by our staff and the patient will be contacted to review the concern and triaged accordingly. Presently, we are able to book families often within 2-7 weeks. 

 

Notice to referring providers & patients: 

We understand that accessing care for lactation supports in our city can be challenging at times. However, we do ask that you be mindful of these limited resources by not referring patients to multiple clinics at one time. One family being seen at multiple clinics for breastfeeding support ultimately takes spaces away from other families and ultimately increases wait times for everyone. Please review the options with your clients/patients and start with one option at a time. Thank you for understanding.
